There's a difference between general, sketchy and nsfw. Turning of "NSFW" doesn't mean that "sexy women" disappear. If you don't want to see those women at all, you have to turn off NSFW and sketchy as well.
What's left should be images of people who are safe for work in any way. If not, you should report the image, so that mods can check it.
